How to complete the European Repair Information Form
A practical sequence for repair businesses

Identify the repairer
Enter the business or repairer name, geographical address, telephone number and email. Stable details can optionally be stored in this browser.

Identify the product
Add the product type, model and, where useful, a serial number or IMEI. Describe the fault and proposed repair clearly.

State the price
Enter the total price where known. Otherwise explain the calculation method and provide a maximum repair price.

Check and provide the form
Review the timing, temporary replacement and ancillary services, then download the PDF and give it to the consumer before the repair contract is concluded.
Which repair price should be entered?
Use a fixed total price when the final amount is known. If it cannot reasonably be calculated in advance, state both the calculation method and the maximum repair price. List transport, installation or other ancillary services separately.

The total amount payable by the consumer, including VAT where applicable.
Calculation method
For example, labour hours × hourly rate + replacement parts used.
Maximum amount
The upper limit the repair price will not exceed under the offer.

Under Directive (EU) 2024/1799, use of the standard form is voluntary for repairers. If a repairer chooses to provide it, the information must be complete and accurate and the stated repair conditions remain binding during the validity period. National implementation rules may add procedural detail.

The form records the repairer’s identity and contact details, the product and defect, the suggested repair, a fixed price or a calculation method with a maximum price, completion time, temporary replacement options, handover location and ancillary services. It is pre-contract information, not a universal repair contract or product intake form.
Directive (EU) 2024/1799 was adopted on 13 June 2024, entered into force on 30 July 2024 and requires EU Member States to apply national implementing rules from 31 July 2026.
